Kinematic Alignment Knee Replacement in Chennai

Restores your knee’s own pre-arthritic alignment instead of forcing every patient to a standard mechanical axis.

Dr. Pankaj Hingarh, MBBS, MS (Orthopaedics) T. Nagar, Chennai Digital X-ray on site

Conventional knee replacement aligns every implant to the same neutral mechanical axis, regardless of the knee it is going into. It is a sound, well-proven approach — yet roughly one in five patients still reports that the replaced knee never quite feels like their own.

Kinematic alignment takes a different starting point. It reconstructs the knee you had before arthritis wore it down, restoring your individual joint line and the natural axes about which your knee actually rotates. The ligaments are then left alone rather than released to accommodate a standard cut.

Dr. Hingarh offers kinematic alignment for patients whose priority is a knee that feels natural in daily movement — sitting on the floor, kneeling at prayer, walking on uneven ground. It does not suit every knee, and the assessment at consultation determines whether your anatomy is appropriate for it.

Kinematic alignment may suit you if

  • You want a knee that feels like your own rather than merely pain-free
  • Your daily life involves floor sitting, kneeling or squatting
  • You have a constitutionally bow-legged alignment that has always been present
  • You are relatively young and active for a knee replacement
  • A previous replacement on the other side felt stiff or artificial

What makes it different

1

Your anatomy sets the target

Bone cuts are referenced to your pre-arthritic joint surface, reconstructing the joint line where it originally sat rather than moving it to a population average.

2

No ligament release

Because the implant matches your anatomy, the collateral ligaments are left at their natural tension. Avoiding release is a large part of why these knees often feel more normal.

3

Rotational axes respected

The femoral component is set to the knee’s own flexion–extension axis, which is what governs how the joint tracks through movement.

4

Robotic assistance where indicated

Kinematic targets are tighter than conventional ones, so robotic or navigated instrumentation is often used to hit them reliably.

Questions

Kinematic Knee Replacement — your questions answered

Still unsure about something? Call +91 99622 45671 and ask.

Is kinematic alignment safe compared with conventional alignment?
Published mid-term results show comparable implant survival with equal or better patient-reported outcomes. Long-term data beyond fifteen years is still accumulating, which is why we discuss the evidence honestly at consultation rather than presenting the technique as strictly superior.
Can I sit cross-legged after kinematic knee replacement?
Many patients do regain the ability to sit cross-legged, and preserving deep flexion is one of the reasons this technique is chosen. It is not guaranteed — pre-operative stiffness and body build both influence the result.
Is kinematic alignment suitable for every knee?
No. It depends on your bone shape, the state of your ligaments and how much deformity has developed. Knees with significant deformity or previous trauma around the joint are often better served by conventional alignment. The assessment at consultation decides it.
