Cartilage Regeneration Treatment in Chennai

Biological repair of focal cartilage defects in younger knees — buying decades before a replacement is ever discussed.

Dr. Pankaj Hingarh, MBBS, MS (Orthopaedics) T. Nagar, Chennai Digital X-ray on site

Articular cartilage — the glassy white surface that lets a joint glide without friction — has no blood supply, and therefore almost no capacity to heal itself. A small focal defect in a thirty-year-old knee will not repair on its own, and left alone it enlarges, taking the surrounding cartilage with it.

Cartilage regeneration techniques exist to interrupt that trajectory. In a young patient with a discrete, well-contained defect — typically from a sports injury or a fragment of bone and cartilage that has broken away — biological repair can restore a working surface and postpone or prevent joint replacement altogether.

The critical distinction is between focal damage and generalised arthritis. These techniques work for a localised pothole in an otherwise good road. They do not work for a road that has worn out everywhere, and for that reason a careful assessment of the whole joint always comes first.

Suitable candidates typically have

  • A single, well-defined cartilage defect rather than widespread arthritis
  • Age generally under 50 with good bone quality
  • Pain localised to one spot in the knee, often with catching or locking
  • A known injury, osteochondral fragment, or osteochondritis dissecans
  • A stable, well-aligned knee — or deformity that can be corrected at the same time
  • Willingness to follow a protected weight-bearing protocol for several weeks

Techniques we use

1

Microfracture and marrow stimulation

Tiny perforations in the underlying bone release marrow cells into the defect, which form a repair tissue. Best suited to small defects; the repair is fibrocartilage rather than true hyaline cartilage.

2

Osteochondral transfer (OATS)

Cylindrical plugs of healthy bone and cartilage are taken from a non-weight-bearing part of the joint and transferred into the defect, restoring genuine hyaline cartilage.

3

Biological augmentation

Scaffolds, PRP and bone marrow concentrate are used to improve the quality and durability of the repair tissue formed.

4

Correcting the cause

If malalignment is overloading one compartment, the cartilage repair will fail unless the alignment is corrected too. An osteotomy is performed alongside where indicated.

Questions

Cartilage Regeneration — your questions answered

Still unsure about something? Call +91 99622 45671 and ask.

Can damaged knee cartilage really grow back?
Not spontaneously. These techniques stimulate or transplant repair tissue to fill the defect. Microfracture produces fibrocartilage, which is more durable than nothing but less durable than the original. Osteochondral transfer restores genuine hyaline cartilage in the transferred plug. Neither is a complete biological reset, and we describe them in those terms.
Am I too old for cartilage regeneration?
These procedures work best under about fifty, in a joint that is otherwise healthy. Beyond that, and where wear is widespread, the results fall away and joint replacement or non-surgical management usually serves better.
How long is recovery after cartilage repair?
Longer than most patients expect. Protected weight-bearing on crutches is typically needed for six weeks while the repair tissue matures, with return to sport at around six to nine months. The protocol is demanding and the result depends heavily on following it.
Will this prevent me needing a knee replacement?
It can delay it substantially, and in a proportion of patients avoid it. It is not a guarantee. The honest framing is that it buys time and function in a young joint that would otherwise deteriorate faster.
