Arthritis & Joint Pain Treatment in Chennai

Structured non-surgical management of arthritis and joint pain — because most joints do not need an operation.

Dr. Pankaj Hingarh, MBBS, MS (Orthopaedics) T. Nagar, Chennai Digital X-ray on site

Most people who come to an orthopaedic clinic with joint pain do not need surgery. They need an accurate diagnosis of which structure is generating the pain, and a plan that is actually followed through. The commonest failure in arthritis care is not a lack of surgical skill — it is a patient given a painkiller and no plan.

Knee osteoarthritis is by far the most frequent presentation, and it arrives earlier in India than in Western populations: partly genetics, partly body weight, partly a lifetime of floor sitting and squatting. Early arthritis responds well to weight management, targeted strengthening and, where appropriate, intra-articular injections.

Dr. Hingarh sets out the full ladder of treatment at the first consultation — what can be done now, what to try if that fails, and at what point surgery becomes the sensible option. You will know where you stand on that ladder before you leave.

Symptoms we assess

  • Knee pain on stairs, squatting or getting up from the floor
  • Morning stiffness in one or more joints
  • Swelling of the knee that comes and goes
  • Grinding or crackling sounds with movement
  • Pain in multiple joints, suggesting inflammatory arthritis
  • Hip or groin pain limiting walking distance
  • Shoulder pain restricting overhead reach

The treatment ladder

1

Step 1 — Diagnosis and staging

Weight-bearing X-rays grade the severity of arthritis and, where inflammatory arthritis is suspected, blood tests identify it. Rheumatoid and gouty arthritis need entirely different treatment from wear-and-tear osteoarthritis.

2

Step 2 — Load management

Every kilogram of body weight translates to roughly four kilograms of force through the knee when walking. Modest weight loss produces disproportionate relief, and specific activity changes reduce peak load through the damaged compartment.

3

Step 3 — Targeted strengthening

Quadriceps and hip abductor strength directly reduce arthritic knee pain. This is prescribed as a specific programme with review, not as generic advice to "do some exercise".

4

Step 4 — Injection therapy

Corticosteroid for acute inflammatory flares; viscosupplementation or PRP for mechanical, degenerative pain. Each has a defined indication rather than being offered indiscriminately.

5

Step 5 — Surgery, only when earned

Joint replacement is discussed when the joint is structurally worn out and non-surgical measures have genuinely been exhausted — not before.

Arthritis & Joint Pain — your questions answered

Still unsure about something? Call +91 99622 45671 and ask.

Can knee arthritis be cured without surgery?
The cartilage damage itself cannot be reversed, but the pain and disability from it very often can be controlled without surgery — sometimes for many years. Weight management and quadriceps strengthening have the strongest evidence of anything available, including most medications.
Are steroid injections in the knee harmful?
Occasional corticosteroid injections for genuine inflammatory flares are safe and effective. Repeated frequent injections — more than three or four a year into the same joint — are associated with cartilage damage, so they are used with restraint and for a defined reason.
Which is better for knee arthritis, PRP or hyaluronic acid?
Current evidence tends to favour PRP in younger patients with early to moderate arthritis, while hyaluronic acid remains a reasonable option in moderate disease. Neither works well in end-stage, bone-on-bone arthritis, and we will say so rather than take your money for it.
Does cold weather or air conditioning worsen joint pain?
Many patients report more stiffness in cold or damp conditions and the association is well documented, though the mechanism is not fully explained. It reflects variation in symptoms, not accelerating joint damage.
