Hip Pain Treatment in Chennai

Groin pain, stiffness and limping — including avascular necrosis, which in Chennai affects a much younger group than most people expect.

Dr. Pankaj Hingarh, MBBS, MS (Orthopaedics) T. Nagar, Chennai Digital X-ray on site

Hip pain is the most commonly misattributed symptom in orthopaedics. True hip joint pain is felt in the groin, often radiating to the front of the thigh and sometimes as far as the knee — which is why a number of patients arrive convinced they have a knee problem. Pain felt over the side of the hip or the buttock is usually something else entirely: trochanteric bursitis, or referred pain from the lower back.

In Chennai, a large share of hip replacements are performed not for age-related arthritis but for avascular necrosis — collapse of the femoral head after its blood supply is interrupted. It affects a much younger group, often men in their thirties and forties, and is strongly associated with prolonged steroid use and alcohol.

That matters because the window for saving the hip is narrow. Caught before the femoral head collapses, core decompression can delay or avoid replacement in a young patient. Caught after, replacement is the reliable answer. Early assessment is the whole game.

Our approach

How the hip is assessed

01

Confirm it is the hip at all

Hip and back pain are routinely mistaken for one another. Rotation tested with the hip flexed reproduces true joint pain and is the quickest way to separate the two.

02

MRI where necrosis is suspected

Avascular necrosis is invisible on early X-rays. In a younger patient with groin pain and a history of steroid or alcohol use, an MRI is warranted — it is the one situation where scanning early genuinely changes the outcome.

03

Preserve the hip if the window is open

Before the femoral head collapses, core decompression with biological augmentation can buy a young patient years. That option closes once collapse occurs.

04

Bearing surface matched to age

Younger, more active patients are given highly cross-linked polyethylene or ceramic bearings chosen for wear resistance over decades, because a hip replaced at forty will need to last.

Questions

Hip — common questions

Anything not covered here, call +91 99622 45671.

Is my groin pain coming from my hip or my back?
Groin pain that worsens when the hip is rotated, and difficulty with socks, shoes or getting into a car, point to the hip joint. Pain over the side of the hip or buttock, especially with tingling down the leg, more often comes from the back. Examination separates them quickly.
What causes avascular necrosis of the hip?
The blood supply to the femoral head is interrupted and the bone dies and collapses. The commonest associations are prolonged steroid use, significant alcohol intake, sickle cell disease and previous hip trauma. In a proportion of cases no cause is found.
Am I too young for a hip replacement?
Modern implants show survival above 90% at 15 years and bearing surfaces have improved further. In a patient operated in their forties a revision at some point should be anticipated and planned for — but living two decades in severe pain to postpone it is rarely the better trade.
Should an elderly hip fracture be replaced or fixed?
For displaced femoral neck fractures in older patients, hemiarthroplasty or total hip replacement is usually preferred over fixation, because it allows immediate weight-bearing. That is the single most important factor in surviving a hip fracture well.
